Appointments/Cancellations
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.
We value your time and the time of our providers. When you schedule an appointment, that time is reserved exclusively for you.

Cancellation/Rescheduling: We require at least 24 hours' notice for any cancellation or rescheduling of appointments. This allows us to offer the time slot to another patient who may be in need of care.

Late Cancellation Fee: Appointments canceled or rescheduled with less than 24 hours' notice may be subject to a late cancellation fee of [e.g., 20% of the appointment fee]. This does not pertain to members.

No-Show Policy: If you fail to attend a scheduled appointment without any prior notice ("no-show"), you will be charged a no-show fee of [e.g., $75.00. Members may have the visit counted against their membership benefits if applicable, or be charged a flat no-show fee.

Repeated Occurrences: Repeated late cancellations or no-shows may result in a review of your membership status or ability to schedule future appointments. We understand that emergencies happen, and we encourage you to communicate with us as soon as possible if an unforeseen circumstance prevents you from keeping your appointment.
Emergencies
Siloam Health & Associates provides comprehensive primary and functional medical care, but we are not an emergency care facility.

For Life-Threatening Emergencies: If you are experiencing a life-threatening medical emergency (e.g., severe chest pain, difficulty breathing, major injury, severe bleeding, signs of stroke, poisoning), immediately call 911 or go to the nearest emergency room. Do not attempt to contact Siloam Health & Associates for these situations.

Urgent Care Needs (Non-Life-Threatening): For urgent but non-life-threatening concerns that arise outside of regular office hours (e.g., sudden onset of fever, minor injury, acute illness), you can contact our office directly at 850-206-5402.
Members: Our DPC membership provides direct access to your provider via secure messaging or phone for urgent issues. We will guide you on the best course of action, which may include self-care advice, a telehealth visit, an urgent in-office visit, or a recommendation to visit an urgent care center. - Non-Members: Non-members seeking urgent care may need to utilize local urgent care clinics or emergency rooms as we primarily serve our enrolled members.

Hospitalizations: In the event you require hospitalization, we will coordinate with the hospitalist team to ensure continuity of care and assist with your discharge planning. While we do not have admitting privileges, we will serve as your primary care advocate.

After-Hours Communication: For non-urgent questions, please use our patient portal or secure messaging system. We will respond during regular business hours. For urgent member concerns, please follow the instructions provided for after-hours contact.

Limited Capacity: DPC practices have smaller patient panels, allowing for more personalized care, and enrollment may be limited.

The Siloam Health & Associates accepts cash and personal checks as well as the following credit cards:

We also accept these payment options :
- Siloam Health & Associates operates as a cash-based medical practice, committed to transparent and straightforward pricing. We do not bill or accept payments from any health insurance providers, including Medicare or Medicaid. This model allows us to focus entirely on your care, eliminating the administrative burdens and restrictions imposed by insurance companies.

Membership Fees: Our primary care services are offered through a simple, affordable monthly or annual membership fee. This fee covers unlimited office visits, direct communication with your provider (via text, email, and phone), annual physicals, chronic disease management, and in-office procedures that fall within the scope of primary care. Specific membership tiers and pricing are available on our website and during your initial consultation.

Functional Medicine Services: Functional Medicine consultations, specialized testing, and advanced therapies are typically billed separately from the core DPC membership and will be discussed and agreed upon with you in advance. We will always provide clear pricing for these services.

Payment Due Date: Membership fees are due on the 1st of each month/quarter/year. Payments for additional services (e.g., labs, supplements, specialized procedures) are due at the time of service unless prior arrangements have been made.

Payment Methods: We accept major credit cards, debit cards, and ACH transfers.

Late Payments: Unpaid balances may incur a late fee of $35 per month if past 5 days late, and it could lead to a temporary suspension of services until the account is brought current. Persistent non-payment may result in termination of your membership.

Receipts for Reimbursement: While we do not bill insurance directly, we are happy to provide you with a detailed superbill or receipt that you can submit to your insurance company for potential out-of-network reimbursement, particularly for labs or services they may cover. Please note that reimbursement is not guaranteed and is solely between you and your insurance provider. We recommend contacting your insurance company directly to understand their policies regarding out-of-network benefits and direct primary care.

The Siloam Health & Associates provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
